This is Archives page template. Just create a page and select it from page Attributes > Templates section from your WordPress dashboard and you are good to go. The basic purpose of this template is to list all your posts month wise and it counts the post publish date for doing this. This page comes as the sample page, if you have inserted a sample data on your site.
